Upper premolar with extensive mesial caries

The patient presented with an upper premolar affected by extensive mesial caries extending subgingivally. A deep margin elevation was performed to facilitate proper isolation and restorative workflow. During endodontic treatment, an apical split was identified and managed, allowing successful completion of root canal therapy. Following obturation, the tooth was restored using EverX composite, chosen for its fiber-reinforced properties to enhance strength and longevity. The case highlights the integration of endodontic management with advanced restorative techniques to preserve function and aesthetics.
